WV Center for End-of-Life Care

The West Virginia Center for End-of-Life Care offers both living wills and medical power of attorney forms on their website at www.wvendoflife.org. In West Virginia, it is easy to complete a living will (also known as advance directives). A person does not need a lawyer. It does not cost anything. The necessary forms can be picked up at any doctors’ office, hospital, nursing home, hospice, home health agency or senior center or downloaded from the Web.
